Regional MDS Coordinator – 3 Skilled Nursing Facilities | Bossier City and Oak Grove, LA

$80,000 – $90,000 per year | Full-Time | Direct Hire

Three buildings, one MDS pro who keeps them all running right. We’re hiring a Regional MDS Coordinator to own assessments and reimbursement across three skilled nursing facilities in Bossier City and Oak Grove, Louisiana.

This is a permanent, direct-hire role. No contract, no per diem.

What you’ll do

• Complete and submit MDS assessments (RAI process) across three SNFs, on time and accurate
• Lead care plan coordination with nursing, therapy, and interdisciplinary teams
• Support PDPM accuracy and reimbursement to protect each building’s revenue
• Stay ahead of compliance and survey readiness
• Coach and support building-level teams on documentation and assessment practices

What you bring

• Active Louisiana RN license (or LPN if you want it open; see my question below)
• Prior MDS Coordinator experience in a skilled nursing setting
• Working knowledge of PDPM, RAI, and Medicare/Medicaid requirements
• Comfort managing a multi-building workload and some travel between sites
