MDS Coordinator (Licensed Nurse)

Tradewater Health & Rehabilitation is a small, family‑owned nursing home committed to meaningful change, higher quality standards, and a renewed focus on honoring the fact that our building is the home of our residents. We are seeking an experienced MDS Coordinator who is a licensed nurse (RN or LPN) and is passionate about accuracy, accountability, and resident‑centered care.

This role is essential to ensuring that every resident receives the quality of care they deserve — and that our documentation reflects the respect we have for their home.


Key Responsibilities

Lead the MDS/RAI process with accuracy, timeliness, and regulatory compliance

Complete assessments that truly reflect each resident’s needs, strengths, and goals

Collaborate with the interdisciplinary team to develop care plans that honor residents’ preferences and dignity

Support quality improvement efforts and help drive positive change throughout the facility

Monitor quality measures and ensure documentation supports high‑quality outcomes

Participate in care plan meetings and communicate clearly with staff and leadership

Promote a culture where documentation, accountability, and resident respect are non‑negotiable

Qualifications

Active Kentucky RN or LPN license (required)

Previous MDS/RAI experience (required)

Strong understanding of PDPM, care planning, and regulatory requirements

Commitment to quality, accuracy, and continuous improvement

Excellent communication and teamwork skills

Passion for supporting a resident‑first culture in a home‑like environment
