JOB TITLE: Conference Facilitator

EMPLOYMENT TYPE: Full-Time

LOCATION: Brooklyn, NY

REPORTS TO: Program Director

 

JOB OBJECTIVE:

The Conference Facilitator will be accountable for providing day-to-day support, leadership, and supporting quality case practice to the Preventive Services program and staff. The role entails strong administrative, engagement, and management skills including strategic planning, resource allocation, and coordination of government agency requirements. Tasks include facilitating and tracking all types of conferences along with the Family Assessment and Service Plan (FASP) reports, as per ACS, coordinating with families, staff, and collaterals by using the Family Team Conference Model to ensure the safety and well-being of each child in the home A successful person in this role will be organized, professional, empathetic, and detail oriented, will thrive under pressure, and be willing to work on multiple projects.

 

A commitment to immigrant, refugee and low-income communities will also be essential for success in this role as the facilitator will be assisting the staff and clients in navigating the systems and connecting families and staff with culturally appropriate services to strengthen the dynamics of the family as a whole using an Evidence and strength-based approach. aafscny.org

 

PRIMARY DUTIES & RESPONSIBILITIES:

Conference Facilitation

·         Schedule and facilitate all Family Team Conferences (FTCs) between families and Case Planners (CP) or Therapists.

·         Maintain detailed notes of each FTC and upload into Preventive Organization Management Information System (PROMIS) and Connections

·         Support pre-facilitation meeting with Supervisor, CP or Therapist, and Case Aide (CA) in preparation for FTC and provide feedback as needed.

·         Participate in all Elevated Risk Conference (ERC) and Child Safety Conferences for each team

·         Ensure FTC is conducted within 4 weeks of the Family Assessment and Service Plan (FASP) due date and run FTC Aggregate Case Summary Case Details

·         Communicate with the Quality Assurance and Improvement (QA/QI) specialist about FTC updates related to Preventive Measurement Assessment System (PAMS) scoring

·         Advocate for higher level services needed for cases/clients

·         Support Supervisor & Case Planner or Therapist during post-conference debrief, and conduct follow up with families and Unit to ensure action plan is complete, and action steps are taking place in a timely manner.

·         Ensure FTC Action Plans are in alignment with progress notes, FASPS & model requirements.

·         Identify and share culturally sensitive services and referrals

·         Communicate with other Conference Facilitators to ensure full coverage of conferences. Participate in weekly supervision and monthly team meetings

·         Appropriately follow incident reporting procedures

o    Cultivate relationships on behalf of and in support of the organization in partnership with Senior Leadership and the Resource Development team. This includes being sensitive to and aware of others, providing every client, partner and participant with high-quality service, professional communication, and follow through, and always keeping Senior Leadership and the Resource Development team aware of any opportunities, challenges, or updates.

o    All team members are expected to support the Resource Development team with any requests in a timely manner, including, but not limited to: grant applications, reports, attendance at meetings, hearings, or other events, and sharing of AAFSC social media, flyers, and other externally-facing communications.

o    Demonstrate a willingness to respond to evolving community needs, including, but not limited to, making referrals in light of community needs, supporting with the development of and implementation of new initiatives or programming, and, raising awareness of new or evolving needs.

o    Always act with integrity, positivity, dedication, and professionalism within the office and externally. This includes, but is not limited to, email communication, verbal communication, communication with colleagues and peers, communication with clients, or communication with external partners or agencies.

 

Other duties & responsibilities

·         Participate in weekly supervision and monthly team meetings.

·         Be willing to travel to family's homes to conduct conferences.

·         Willingness to work some evenings and weekends.

·         Strong verbal and written communication skills

·         In support of Case Planners or Therapists, complete home visits when needed and as assigned by Program Director or Supervisor.

 

BACKGROUND & POSITION REQUIREMENTS:

·         MSW or equivalent human services graduate degree and two years of casework experience or one year of group facilitation or supervisory experience.

·         Strong facilitation and interpersonal skills.

·         Strong verbal and written communication skills.

·         Bilingual or multi-lingual fluency in Arabic, Bangla/Bengali, Russian, or Uzbek strongly preferred.

·         Knowledge of child welfare system and regulatory procedures for child welfare facilitation

·         Knowledge of Evidence Based Models.

·         Strong understanding of systematic approaches for engaging families in team conferences.

·         Ability to develop and sustain relationships with programs and clients.

·         Experience working with various stakeholders and developing collaborative relationships.

·         Strong attention to detail and maintain accurate well-organized records.

·         Ability to work independently and in teams.

·         Job contingent on pre-employment background check and fingerprint clearance

·         U.S. Work Authorization is required
