ACTT Team Leader (LCSW / LCMHC / LMFT)

Schedule: Mon–Fri, 8:00–5:00
On-call: 24/7 rotation shared across 6-8
What this job looks like (week to week)
- Caseload: 8–12 adults with serious mental illness (SMI) + substance use
- Field work in homes, shelters, hospitals
- Office/admin time for supervisions, documentation, etc.
- Team: psychiatry, nursing, therapists, peer support
- Crisis calls: part of rotating 24/7 coverage
What you do
Clinical- Complete biopsychosocial + substance use assessments
- Build treatment plans with the ACTT team

Deliver therapy using:
- Motivational Interviewing
- Cognitive Behavioral Therapy
- Relapse prevention
- See clients in homes, shelters, ERs, and community locations
- Run 2–4 person-centered meetings/week
- Support stabilization (med adherence, coping skills, anger regulation)
- Respond to behavioral health crises in the community
- De-escalate situations
- Guide 6–8 team members on clinical decisions
- Coordinate with psychiatry, nurses, employment specialists, peer support
- Review documentation to meet Medicaid + ACTT standards
Requirements
License (North Carolina)-
LCSW, LCMHC, or LMFT
- ≥1 year working with adults (paid or volunteer)
- Direct work with SMI, substance use, or crisis populations
- Driver’s license + daily travel
- Lift up to 25 lbs
