About the Role: Speech and Language Pathologist (SLP)
As a Speech and Language Pathologist at Sprout Family Health, you’ll be part of a multidisciplinary team redefining family-centered proactive healthcare. Working alongside a team led by a Nurse Practitioner and supported by a Health Navigator (Registered Nurse), as well as specialists in physical, functional, and mental health, you’ll deliver personalized, holistic care that supports the health and wellness goals of our families.
This role is focused on providing screening, parent education, and professional guidance. In this position, you will work with families to identify early signs of speech, language, or communication difficulties through screenings, offering informed recommendations and guidance on whether further treatment may be beneficial. Additionally, you’ll empower parents with education and resources to support their child’s communication development. When intervention is indicated, you will facilitate referrals to community-based SLPs for comprehensive assessment and therapy. Ideal candidates will have strong interpersonal skills, a collaborative approach, and a passion for supporting families through education and informed recommendations.
Our team-based model emphasizes early intervention, seamless access to care, and equipping families with the knowledge and tools they need for lasting health. If you’re passionate about making a difference in the lives of young families and thrive in a collaborative, proactive healthcare environment, we’d love to hear from you!
