This employee will work within a 24-hour intensive supported residential housing model that will serve a total of 2 women Individuals served will have a confirmed Developmental Disability, Mental Health Diagnosis, and challenging behaviors including: self-harm, property destruction, verbal aggression. They will provide delivery of direct support services to both residents within the housing model and ensure the physical and emotional well-being and safety/security of all individuals within the residential site and in the community in alignment with YWCA Hamilton’s Mission and Vision Statements including ARAO philosophies.
KEY RESPONSIBILITIES:
· This is unique funding model to provide the appropriate services and supports to meet the complex needs of the residents.
· This employee will work in cooperation and alongside with other Community Agency Staff, YWCA Hamilton Staff, including Assigned Program Supervisor, full time, part-time and relief team members.
· They will perform all duties and responsibilities in a productive, professional and cooperative manner which will contribute to the overall goals of the partnership and DS Department and in keeping with general policies and guidelines of YWCA Hamilton.
· Ensure the physical and emotional well-being of individuals we support is maintained.
· Follow and adhere to QAM compliance and Legislation 299/10 standards.
· This employee will provide delivery of direct support services to individuals we support within their residential housing model.
· Support services in all areas of daily living within our service plan domains including but not limited to: bathing, hygiene routines, medication administration, medical/dental coordination, ISP development, meal preparation, grocery shopping, chore lists/housekeeping, laundry, nightly safety inspections, advocacy, and in-house recreation opportunities and community outings that promote social inclusion.
· Provide supervision and ensure safety of residents during their shift. Support services offered will maintain or increase individual’s independent living skills and personal growth.
· In cooperation with program supervisor will perform all duties and responsibilities in a productive, professional and cooperative manner which will contribute to the overall goals of the partnership and DS Department and in keeping with general policies and guidelines of YWCA Hamilton.
· Ensure the physical and emotional well-being of individuals we support is maintained.
· Assist residents with ADL’s, meal preparation, grocery shopping, menu planning, medication administration, assistance with hygiene routines, housekeeping, laundry, community safety skills, transportation, reducing risks to harm, community outings, recreation and social opportunities, and medical/specialist appointments.
· Must attend and participate in regular team meetings in order to communicate and share information including achievements, progress, concerns and generate solutions.
· Attend on-going professional development opportunities offered to program staff.
· Contribute to the development, planning, implementation of day/evening and weekend in-house and community based social/recreational activities.
· Program and agency policies are adhered to by staff and reviewed regularly.
· All necessary administrative/documentation tasks are performed: FT case manager monthly summaries, ISP & Reviews completed by Case managers, ISPs, BSPs & PRN protocols adhered too, data collection, teaching & wellness plans, daily communication logs, checklists, chore lists, inspections, incident reports, fire drills or other reports/tasks as assigned.
· Ensure that housekeeping/cleanliness/maintenance of program areas or residents’ accommodations standards are met.
· Establish positive professional relationships with staff, individuals, families, and other community service agency providers.
· Provide accurate and timely reports as required.
· Ensure YWCA Hamilton workplace health and safety policies and procedures are followed at all times and act to reduce workplace hazards.
· Report all workplace hazards and follow YWCA Hamilton health and safety policies.
The physical demands of the role may include but are not limited to lifting (up to 25kg), bending, twisting, reaching, sitting or standing for extended periods, walking, pushing, pulling and moving constantly throughout the shift.

QUALIFICATIONS
· Minimum Education: SSW or enrolled for completion, DSW Apprenticeship or enrolled, CSW or other human service field
· 1-2 Years Part Time or Relief work experience in a 24hr residential/SIL setting
· Previous work experience with individuals with Mental Health Diagnosis and Challenging Behaviors
· Emergency First Aid/CPR Certification
· Mental Health First Aid Certification
· NCI Certification
· QAM/Abuse Training, Fire Safety
· ARAO Training
· Must have demonstrated excellent verbal and written skills
· Ability to work independently and as a part of the team
· Must be able to work all the rotating schedules as assigned (see schedule)
· Proficiency with MS Office Applications including Word & Outlook
· Working from a feminist, anti-oppressive/anti-racist perspective
