Overview: The nurse practitioner provides a wide range of comprehensive primary health care services such as: physical and mental health assessments, ordering and interpreting diagnostic imaging and laboratory tests, diagnosing and treating chronic and acute illness, reviewing disease prevention and screening guidelines, and  referring to specialist care.

We are currently recruiting for 1 Part time, Permanent Nurse Practitioner. This is a replacement position within the organization. 

Responsibilities:

  • Practice to full scope of NP-PHC practice including: assessment, diagnosis, treatment, follow-up, education, consultation and referral 
  • Physical and mental health assessments, order appropriate diagnostic imaging and laboratory tests,
  • Diagnose and treat chronic and acute illness, and review disease prevention and screening guidelines
  • Collaborate with client’s case management team  
  • Attend case consults
  • Attend team meetings
  • Refer to specialist care
  • Provide expertise and guidance in policy and practice discussions
  • Stay informed of current best practice and evidence-based guidelines
  • Assist with the review and development of medical directives 
  • Provide consultation and education to the clinic RNs as required 
  • Participate in team and agency meetings 
  • Contribute to capacity building and professional development of agency staff  
  • Ensure that healthcare services align with best practices and current evidence based literature 

Qualifications

  • Completion of a Master’s-level nurse practitioner program and nurse practitioner specialty certification.
  • Supervised clinical placements with a nurse practitioner or physician over the course of two years.
  • Registered with the College of Nurses of Ontario (CNO) as a Registered Nurse (RN) in the Extended Class  
  • Must hold a nurse practitioner specialty certification
  • Completion of a CNO-approved controlled substances education program 
  • Primary Health Care (PHC) Certificate 
  • Able to practice autonomously and as part of an interprofessional team 
  • Excellent clinical, organizational, advocacy, leadership, collaboration, and interpersonal skills  
  • Strong computer skills and familiarity with electronic medical records systems
