About the Opportunity

The Case Manager for the Heart to Home Program, leads, develops, and supervises all care planning activities for children and youth in The Link Healing Home. They ensure care providers are supported in delivering quality care and also contribute to the continuous development of the program.

Duties and responsibilities are carried out within a trauma and resiliency informed framework that promotes The Links’ Therapeutic Model of Care, including the agency mission, guiding principles and the Circle of Courage® values. Duties are performed within a team environment that focuses on establishing collaborative and trusting relationships with one another and with agency clients. Providing culturally competent services and ensuring client safety, choice and control are priorities at The Link.

PRIMARY RESPONSIBILITIES: 

Care Planning
  • Ensures a holistic, systems and family-centered approach to planning.
  • Provides information to The Link Centralized Intake and Assessment for decision making regarding placement to ensure resources meet needs of youth.
  • Participates in placement process, when possible, to facilitate smooth transition for youth.
  • Works collaboratively with youth, families, care providers and placing agency worker in development of assessments, plans and interventions.
  • Liaises with internal clinical service team and external systems for the purpose of information gathering, scheduling of Care Plan meetings and associated tasks.
  • Monitors and re-evaluates the Care Plans on a quarterly basis ensuring identified actions to meet goals are implemented.
  • Maintains and/or develops plan for family contact.
  • Proactively facilitates transition, discharge and age of majority planning for every youth in placement.
  • Ensures appropriate and timely record keeping for individual caseload.
  • Provides guidance and direction related to youth experiencing crisis or when emergency planning is required.
  • Participates in providing after-hours support to the program through an on-call system.
  • Other duties, as required.
Parent Support / Heart to Home 
  • Participates in the provision of orientation of the Link and the Heart to Home program for all new parents.
  • Ensures the completion of annual foster home licensing document and foster parent assessment in order to evaluate foster parents and make recommendations in accordance with existing provincial regulations for ongoing continuation of maintaining exclusive the Link foster home licenses.
  • Complete home studies as required for newly recruited foster parents.
  • Ensures quality care is provided by parents by maintaining regular contact (via telephone, home visits, meetings, case reviews, foster home reviews, reviewing incident reports, providing crisis management, development and ongoing consultation regarding the care plan)
  • Reviews incident reports and provides crisis management and debriefing.
  • Provides ongoing consultation regarding the care plan expectations and ensures related professional development goals for parents are identified and implemented by the parents.
  • Oversees parents’ use of their respite team by ensuring respite workers are included as part of the team and are used appropriately to support foster home operations and day-to-day childcare plans.
  • Monitors per diem expenditures, including the use of respite/support worker hours, mileage, clothing, allowance and activity funds.
  • Ensures appropriate and timely record keeping.
  • Participates in providing after-hours support to the program through an on-call system.
  • Other duties, as required.
Ongoing Program Development & Quality Assurance Initiatives
  • Attends team meetings, and children and youth care planning meetings.
  • Attends relevant training as required by agency policy or supervisor.
  • Participates in clinical consultation with Clinical Manager and/or delegate.
  • Liaises with other agency programs.
  • Participates in agency committees as approved by the Coordinator, Healing Homes.
  • Attends mandatory agency and program events.
  • Represents The Link and Healing Homes at agency and community functions.
  • Other duties, as required.

QUALIFICATIONS:

  • Post-secondary degree in human service field. Bachelor of Social Work degree preferred.
  • 3 years’ experience in one or more of the following areas: child welfare, foster care, residential care, case management, risk assessment and safety planning.
  • Gather relevant information, recognize patterns and themes in the data, generate and consider multiple alternatives and make decisions in a timely manner;
  • Analytical and critical thinker, particularly in regard to engaging and planning with the youth.
  • Must possess a valid 5F driver’s license, provide a current driver's abstract (subject to approval) and proof of vehicle insurance.
  • Strong written and oral skills.
  • Microsoft Office at an Intermediate level.
  • Satisfactory Criminal Record Check with Vulnerable Sector Search (check must be complete through RCMP)
  • A satisfactory Child Abuse Registry check with acceptable Prior Contact checks and a clear Adult Abuse Registry check, all done within the last 3 months.
  • ASIST Certificate (Applied Suicide Intervention Skills Training) preferred.
  • De-escalation training 
  • Willingness to obtain training as per The Link's policy/agency required training.
  • Knowledge of Indigenous cultural practices and experience working with Indigenous youth, agencies and community organizations will be considered an asset.
  • Knowledge of systems for children in care, school, family functioning and child welfare.

HOURS OF WORK: 35 hours per week; Currently, the hours of work for this position are Monday to Friday from 8:30am to 4:30pm

EMPLOYMENT TYPE: Full-time

SALARY RANGE: $42, 320.00 - $63, 480.00 per year *dependent upon qualifications and experience*
