Pay Grid: BCNU Nurse Grid (RN 3/LPN Wage Schedule) ($32.84 to $45.03 per hour + $2.15 hourly premium)

Summary of Duties:
Under the direction of the Home Care Manager, the Registered/Licensed Practical Nurse (RN/LPN) - Home Care will support clients and their families in the development and implementation of the client’s care plan and will assist in connecting them with appropriate community support and resources.
Key Deliverables and Expectations:
- Perform all duties and responsibilities in accordance with British Columbia College of Nurses and Midwives (BCCNM) standards and guidelines and Lil’wat Nation formal policies, standards, procedures, and standard practice.
- In consultation with Nurse Manager performs duties as directed by Nurse Manager and implements and evaluates programs.
- Establish a therapeutic relationship with the client or family using interpersonal and interviewing techniques to ensure their choice and autonomy in decision-making and care planning.
- Conduct structured nursing assessments and reassessments of clients who are eligible and agree to the program to determine services required. Encourage participation of client, family other caregivers and/or care providers where possible in the assessment process.
- Use their professional knowledge to work in partnership with the client and family to establish wellness goals that are safe, realistic, and reasonable.
- Use a holistic approach in providing client care, case management, care planning, referrals, and links to other services to support client in optimal wellness outcomes.
- Incorporate the principles of Two-Eyed Seeing (using traditional and Western approaches) to deliver direct care, support clients in managing their own care and assist them in navigating the various services available within the community settings.
- Promote the Home Care Program and educate community about the program and services.
- Clinical support and educational support for Health Care Assistants.
- Assist the client and their family in accessing medical equipment and supplies.
- Evaluate own practice and use professional consultation, training, and professional resources to maintain strong practice standards and the provision of quality care to community members.
- Administration tasks that assist in the delivery of the Home Care Program.
- Perform other related duties as requested.
Experience and Attributes:
- Current practicing registration, in good standing, with the British Columbia College of Nurses and Midwives (BCCNM).
- Degree in Nursing from a recognized university is an asset.
- Current CPR/AED for healthcare providers.
- Demonstrated knowledge or experience working with First Nations health issues and health systems.
- One year of experience working in a community health setting and wound care setting or an equivalent combination of education training and experience.
- Experience or working knowledge of elder health, chronic disease, developmental disability, and mental health nursing practice.
- Experience in providing education around chronic diseases, advanced care planning and education around palliative care.
- Experience working in a rural setting will be a benefit.
- Case management experience and experiences as a liaison with other health services will be advantageous.
- Palliative care experience is an asset.
- Foot care certification a benefit.
- Valid BC Class 5 Driver’s License, a clean Driver’s Abstract, and a reliable vehicle.
- Must be able to provide a clear Criminal Record Check, including the vulnerable sector.
