Job Summary: Brockton Home Health Agency is looking for a Lead Home Health Clinical Manager, inspire and motivate clinical manager to run our Mass Health/Medicaid, Medicare and AFC program.

• Monday - Friday 9:00AM – 5:30PM day shift schedule.

As a Clinical Manager your day may look like this!

·        The Clinical Manager coordinates and oversees direct care patient services provided by clinical personnel

·        Responsible for and oversees the delivery of care to Brockton HHA patients

·        Assisted and assign all referrals to the proper discipline and initial visits are done timely.

·        Accountable to ensure patients meet admission criteria and make the decision to admit patients to service

·        Assist and assigns appropriate clinicians to a case, as needed

·        Assist with clinical questions from insurance providers

·        Manage a team of Clinicians (PT’s, RN’s and LPN’s)

·        Coordinates communication between Brockton HHA and insurance providers

·        Provides direct patient care on an infrequent basis and only in times of emergency

·        Maintains relationships with referral/community sources

·        Assist with PA review before submit to insurance providers

·        Communicate with clients and their families to establish care plans and ensure all needs are met.

·        Coordinate with community resources and healthcare professionals to enhance Brocktonhha's brand awareness, attending meetings and connecting with potential referral sources.

Qualifications

·        Registered Nurse (RN) with 5 or more years of experience as an RN.

• Supervisory experience is preferred.
• Passionate about mentoring/ educating others.
• Excellent communication, time management, and organizational skills.
• Ability to work both independently and in a team.
