Registered Nurse *PRN

Key Responsibilities

• Complete patient assessments including OASIS evaluations in accordance with agency and Medicare standards.
• Develop and manage individualize plans of care based on patient goals and clinical findings.
• Provide skilled nursing care, ensuring safety, progress, and dignity for each patient.
• Educate and support families through understanding empathy and guidance.
• Accurately document our visits and maintain compliance with all home health regulations.

Qualifications

• Active RN license in the state of Illinois
• Minimum of one year of clinical nursing experience
• Prior home health and Oasis experience preferred
• Strong organizational assessment and communication skills
• Reliable transportation and a heart for patient centered care
