MDS Coordinator (RN or LVN)

We’re hiring a detail-oriented and compassionate MDS Coordinator to join our team at Liberty Health Care Center. This role is perfect for a licensed nurse with RAC-CT certification who is passionate about ensuring quality care, compliance, and accurate resident assessments.

• Oversee the MDS assessment process to ensure compliance with state, federal, and facility standards.

• Accurately complete and manage resident assessments and care plans.

• Collaborate with interdisciplinary team members to ensure quality, individualized resident care.

• Monitor data collection and timely submission of MDS records.

• Educate and guide staff on MDS processes and documentation requirements.

• Assist with audits, reporting, and regulatory compliance activities.

• RAC-CT Certification required.

• Current and active RN or LVN license in Texas (unencumbered).

• Current CPR certification (or ability to obtain/maintain).

• Strong knowledge of nursing/medical practices and long-term care regulations.

• Excellent analytical, communication, and organizational skills.

• Ability to make independent decisions, manage priorities, and follow through effectively.

• Physical ability to lift 60–70 lbs and remain on your feet during shifts.

• Positive, team-oriented attitude with strong interpersonal skills.

• Must successfully pass a background check, drug screen, and physical (if required).
