Job Title: Veteran Care Coordinator (Cannabis Clinic Administrator)

Reports To: Senior Manager, Strategic Partnerships

Summary:

The Veteran Care Coordinator is responsible for providing an outstanding patient experience and high quality patient care. The Veteran Care Coordinator supports and meets the needs of  the relevant veteran patient population. Communication and collaboration with patients, Apollo staff, and our allied healthcare partner (The Veteran Farmer) are core expectations of this role.

Essential Responsibilities:

- Liaise with Allied Healthcare partner (The Veteran Farmer)

- Ensure clinic days are organized: i.e. cross check patient files, complete medical history, index all forms into EMR, ensure healthcare practitioners are notified of any important

details, and be available for questions/issues etc.

- Facilitate coverage for patients – i.e. submit registration and supporting documents to Licensed Producer(s), diligent follow up to ensure interim coverage eligibility, submit

required documents to Medavie Blue Cross/Veterans Affairs Canada, arrange for vaporizer coverage when applicable

- Address all patient and allied healthcare partners concerns and questions in a timely fashion

- Support the development and implementation of quality improvement processes, including metrics and performance indicators

- Facilitate patient requests (Licensed Producer changes and/or splits, follow up with Medavie Blue Cross/Veterans Affairs Canada)

- Organize and facilitate clinic days with nurse practitioners and specialist physicians

- Work with specialists to draft/submit Specialist Letters

- Follow up with patients after their appointments (ensure prescription dosages are sufficient, documents are correctly actioned, follow up appointments are scheduled, etc.)

- Provide enhanced education and support to patients as required

- Ensure and track that patients are successfully managing their medication through monthly contact

- Track relevant expiration dates and avoid prescription expirations

- Collaboration with Apollo staff to provide patients with a best-in-class patient experience

- Tracking of relevant metrics and management of various tracking documents

- Identify trends, issues and challenges requiring change in practice patterns or processes

- Ensure professional approach to care at all times while recognizing the unique contribution of each team member

- Contribute to the organization and implementation of Apollo's mission, vision, values and strategic plans

- Other tasks as assigned


Requirements:

- Exceptional attention to detail and strong organizational skills

- Comfortable educating patients on medical cannabis strains, modalities, titration, etc.

- Ability to troubleshoot in a fast-paced environment

- Ability to identify and balance priority tasks, excellent time management

- Knowledge of pharmacology and medical terminology is an asset

- Knowledge of EMR, Microsoft 365, Google Suite, etc

- Comfortable working as part of a team and able to work independently

- Strong interpersonal, communication, and organizational skills; ability to work cross-functionally and collaborate

- Flexible, compassionate, empathetic and patient-centric approach

- Proficient in English (French is an asset) with outstanding written and verbal communication skills

- Strong working knowledge of Licensed Producers, available medication is an asset
