FT Collaborative Care Practitioner – Virtual Women’s Health

We are seeking a highly motivated and empathetic Registered Nurses, Nurse Practitioners, and Clinical Pharmacists to join our clinical team to lead menopause assessments. In this role, you will serve as a primary clinical contact for patients experiencing perimenopause and menopause, providing comprehensive assessments, education, and care coordination to support their well-being.
You will work collaboratively with nurse practitioners, pharmacists, physicians and allied health professionals to ensure patients receive timely, personalized, and effective care.
Key Responsibilities
Conduct Comprehensive Virtual Menopause Assessments:
- Evaluate symptoms, menstrual history, medical and family history, and lifestyle factors
- Use standardized assessment tools to identify menopausal stage and symptom severity
- Screen for associated conditions such as osteoporosis risk, cardiovascular changes, and mental health concerns.
Education and Counselling:
- Provide evidence-based education on menopause, treatment options (including menopausal hormone therapy, non-hormonal options, and lifestyle changes).
- Empower patients with guidance on symptom management, sleep, mood, genitourinary health and nutrition.
Care Planning and Coordination:
- Develop preliminary care plans and flag patients who require medical or specialist follow-up
- Coordinate referrals within Effica’s multidisciplinary care team when needed
- Follow up with patients to assess response to interventions and adjust plans accordingly
Documentation and Communication:
- Accurately document all assessments and care notes in Effica’s technology platform
- Communicate patient needs and updates to care team members in a timely and respectful manner
Continuous Improvement and Quality Care:
- Participate in training and case reviews
- Provide feedback on protocols and tools to improve the menopause care pathway
- Stay up to date with current guidelines and research in women’s health and menopause
Qualifications
- Licensed Nurse Practitioner, Registered Nurse or Clinical Pharmacist in good standing with one or more provincial regulatory bodies
- Minimum 3 years of clinical experience; prior experience in women’s health, virtual care, or telehealth is strongly preferred
- Exceptional interpersonal and communication skills; comfort discussing sensitive health topics
- Strong clinical judgment and ability to work independently within a virtual care team
- Experience with digital health tools and EMRs
- Comfortable navigating health education, lifestyle counselling, and triaging for specialist input
