Job Summary:
The Care Navigator (0.50 FTE) is responsible for for the flow of resident movement through the Home from move-in to internal transfers and discharge of residents. The Care Navigator is responsible to ensure Ocean View is in compliance with all DSLTC timelines and procedures. The Care Navigator plays an integral role in the quality of life for residents. The Behaviour Support Coordinator (0.50 FTE) works directly with the staff and residents to ensure a positive, person-centered approach is used to support residents with behavioural and psychosocial symptoms of dementia (BPSD) through coaching, education, policy development and facilitating care planning.

 

Qualifications:

·         Post secondary education (degree or diploma) required in a health-related field

·         Degree in social sciences an asset

·         GPA Coach preferred

·         Minimum of five (5) years experience in long term care and working with persons with BPSD/responsive behaviours

·         Excellent communication skills

·         Experience providing coaching and education to a multidisciplinary team

·         Ability to manage competing priorities

·         Clear Criminal Records and Vulnerable Sector Check

Hours of Work:

The regular hours of work for this position are intended to be 8:00 am to 4:00 pm Monday through Friday. Given the scope and responsibility of the role, there may be a need to flex hours outside this range.
