Outreach Coordinator - Homeward Bound Oakdale

Prince Albert, SK Full-time $57942.00/year

Full-Time Permanent Salaried (37.5 Hours Per Week)

Responsibilities: The Outreach worker is responsible for connecting clients to supports and community resources in a culturally sensitive manner. Gain & maintain CLSD training and implementation of person-centred supports to ensure a positive and safe environment for all.

Qualifications: 

  • Valid Class 5 Saskatchewan Driver’s Licence with a clean abstract;
  • Practical experience in in the Human Services, supportive housing fields.
  • Experience working with individuals with mental health challenges, substance use concerns, and/or cognitive disabilities.
  • Knowledge and understanding of Indigenous culture.
  • Knowledge of community resources, supports and other agencies.
  • Strong knowledge of Trauma Informed Care, person-centered planning, and strength-based approaches.
  • Maintain professional boundaries in and out of the workplace as outlined in the YWCA Code of Conduct and Ethics Agreement.
  • Strong organizational, administrative, and time management abilities.
  • Excellent communication skills (written and verbal) and interpersonal skills.
  • Proficient in Microsoft Office applications (Word, Excel, Power Point, Outlook).
  • Must be available for the on-call rotation.
  • Must pass Criminal Record Check and Vulnerable Sector Check.
  • Ability to work independently as well as part of a team.
  • Must be able to work flexible hours when needed.
  • Must be a positive role model of life and social skills for clientele.

Basic Duties:

  • Maintain written and computerized records, compile reports, and complete other program documents (including case notes, statistics, letters, etc.)
  • Represent Homeward Bound Oakdale in meetings as required.
  • Uphold and maintain policies and procedures for Homeward Bound Oakdale.
  • Case Plan development, tracking, and execution.
  • Provide a monthly report & annual report to management, and monthly calendar of events for clients.
  • Set long and short-term goals with a timeline for completion.
  • Provide individually tailored services to each client.
  • Ensure all incidents are properly documented and reported to the manager.
  • Provide transportation to client appointments as needed.
  • Occasional work in non-traditional hours as assigned by management.

Program Specific Duties:

  • Develop and coordinate a plan for individuals to transition into their new housing.
  • Develop and implement behavioral support plans in collaboration with management team.
  • Assist in making appointments that enhance the wellbeing of clientele (medical, educational, vocational).
  • Plan and prepare person-centred planning meetings annually.
  • Update case plans with clients quarterly.
  • Establish a positive rapport with landlords in the community.
  • Assist in the implementation of support plans and case-management.
  • Establish the types of supports needed for individuals that have transitioned; areas of improvement, life skills needed, etc.
  • Assist in setting up community supports once transitioned, such as educational applications, counseling, parenting programs, etc.
  • Review all communications logs when coming on shift.
  • Work closely with the Homeward Bound Oakdale Team for dwelling checks, cover off, support transitioning clients, and programming.
  • Include outreach clients in the building in programming.
  • Support staff members at Homeward Bound Oakdale as needed.
  • At all times, be aware of the Homeward Bound Oakdale environment and do your part to ensure it is safe, clean, and welcoming.
  • Meet hourly support requirements for each client on caseload.
  • Follow safety and security policies at all times.
  • Other duties and responsibilities as deemed necessary by management to ensure the effective operation of Homeward Bound Oakdale.
