Monday to Friday 8:00am to 4:00pm with ½ hour paid break
Wage Range: $33.77 up to $38.77 based on
education and related experience.
Position Summary: Under the direction of the Leadership Team, coordinate the operations and activities of Mission Flats to provide healthy, independent housing through intensive support and services to the community with the hope of reducing homelessness. In collaboration with the ASK Wellness Society Officers, Directors, and Coordinators work to ensure standardization of systems, identification of efficiencies, integration of performance improvement strategies, and client and employee satisfaction. Service delivery is based on a client-centered, non-judgmental perspective.

Specific Responsibilities
• Encourage and support Program Participants to live as fully and independently as possible within the local community, providing information, emotional, organizational, and practical support, and training as appropriate.
• Participate in client selection meetings to ensure appropriate program referrals and bed utilization.
• Coordinate Tenant Support Workers to provide individualized support, as needed, to multiple barriered clients and to assist them in maintaining affordable, safe, and supported community housing.
• Conduct client-centered Case Management meetings with all Participants to ensure compliance with program agreement, vocational plan, mental health and wellness plan.
• Liaise with key community caregivers regarding necessary support systems for the clients including financial and life skills programming.
• Provide individualized crisis support and assessment to multi-barrier clients as needed.
• Debrief with Tenant Support Workers and refer to the Leadership Team when required.
• Ensure case notes, statistics and operational information are up-to-date, and appropriately shared with the Society Officers, Directors and Coordinators, Property Services and Finance.
• Responsible for program fee collection and managing vacancy loss.
• Review and approve electronic timecards.
• Organize workflow and ensure that employees understand their duties or delegated tasks and conduct annual Performance Reviews as directed.
• Monitor employee productivity, case notes and provide constructive feedback, training, and coaching by setting clear and strategic objectives.
• Receive complaints, resolve problems, or refer to an appropriate Officer/Director when required.
• Work with the Human Resources Department to ensure compliance with company policies and procedures and refer to any disciplinary actions related to employees.
• Contribute to the protection of Program Participants from abuse, and support Participants when they are in distress as well as motivate them to develop a sense of their own identity, purpose and self-esteem.
• Monitor emergencies that take place such as medical or mental health assistance and complete necessary incident reports and when required, be trained to be the first to respond to an overdose situation with on-the-spot overdose response and support.
• Monitor and move along community members who are loitering in and around the property.

Administrative Responsibilities:
• Assist team with maintaining daily log, medication log/prescriptions (ensure book is updated, all medications are accounted for and follow up is on prescriptions to ensure medication is up to date).
• Complete move in/move out paperwork as required by ASK Property Services Department under supervision from the Leadership Team.
• Maintain complete documentation, statistics, and records in files and on ShareVision database.
• Assist team with required verbal and written reports, case notes, letters, memoranda, and year end reports.
• Maintain files and documentation to the COA Standard.
• When requested, be a member of the COA’s Performance and Quality Improvement team in which you will encourage the organization to use data to identify areas of needed improvement and implement improvement plans in support of achieving performance targets, program goals, client satisfaction, and positive client outcomes.
Qualifications:
· Bachelor of Social Work, Human Service Diploma or equivalent and/or experience with mental health/addiction and homelessness issues.
• Skills in mediating conflict between various interests.
• Ability to work as a team member in a case management setting involving multi-agency support.
• Understands and maintains clientele/worker boundaries.
• Strong working knowledge of community resources.
• Excellent communication, writing, documentation and organizational skills.
• Must pass criminal record check for vulnerable sector and reference screening.
• Mental Health First Aid, Trauma Informed Practice and ASSIST Training an asset.
• Basic First Aid required.
• Must have access to vehicle in good working condition and possess valid driver’s license with relevant insurance to transport clients.
• In-house Violence Prevention and Cultural Awareness Training provided.
