We are seeking a dedicated and experienced Wound Care Nurse Supervisor to join our clinical leadership team. This position plays a vital role in delivering high-quality wound care and ensuring regulatory compliance in a long-term care setting. The ideal candidate will possess a blend of clinical expertise, leadership acumen, and a strong understanding of New York State and federal healthcare regulations.
Key Responsibilities:
Clinical Oversight

Assess, plan, and implement individualized wound care protocols

Conduct wound evaluations including staging, treatment planning, and progress monitoring

Maintain oversight of wound care rounds and adjust treatments as necessary
Supervisory Duties

Lead and support nursing staff in wound care best practices

Train and mentor nursing team members on prevention and treatment procedures

Collaborate effectively with physicians, therapists, dietitians, and other members of the interdisciplinary team
Regulatory Compliance and Quality Assurance

Ensure adherence to CMS, NY Department of Health, and Joint Commission standards

Monitor wound-related outcomes and infection rates

Participate in audits and contribute to QAPI (Quality Assurance Performance Improvement) initiatives
Documentation

Maintain accurate and timely clinical records of wound care assessments and interventions

Ensure documentation supports reimbursement and regulatory requirements, including MDS (Minimum Data Set) documentation
Qualifications:
Licensure and Education
-
Must be a Registered Nurse (RN) licensed in New York State

BSN preferred; Associate Degree in Nursing (ADN) acceptable with relevant experience

Wound care certification (WCC, CWCN, or equivalent) strongly preferred
Experience
-
Minimum of 3–5 years of clinical nursing experience, with at least 2 years in wound care

1–2 years of supervisory or leadership experience preferred

Prior experience in a skilled nursing facility or long-term care setting is essential
