Wound Care Nurse RN or LPN
Demonstrate clinical competence to evaluate the physical, spiritual, psychological, emotional as well as educational needs of residents.
· Order, administer and record medications and treatments in accordance with physician orders and nursing policies and procedures.
· Transcribe/verify physician orders, coordinate testing, treatments, and other procedures as appropriate
· Follow up on the administration of treatments for compliance.
· Evaluate the effectiveness of an implemented treatment plan.
· Provide necessary wound/treatment education to staff.
· Monitor stock levels of medication, medical supplies related to wound care, wound care equipment etc. and to verify they are always maintained on premises to meet residents’ needs.
· Participate in weekly facility wound care meetings.
· Participate in facility monthly QAPI meeting.
· Make rounds with the wound care practitioner weekly
· Assist in orienting new nurses on wound care treatment of our residents with skin altercations.
· Participate in care planning documentation and meeting involving resident wounds and treatment plans.
· Executes interdisciplinary care plan (under minimal supervision) for each resident and assists with review and revision of each care plan as required.
· Observes residents for changes in condition due to drugs, treatment, etc. and reports adverse changes to supervisor.
· Complete new admission nursing skin evaluations, and document in accordance with the facility and regulatory guideline. Document nursing interventions and resident response to care timely, accurately, and in a complete manner.
· Wound Nurse responsibilities may require an adjusted work schedule, overtime and evening or weekend hours to meet facility's needs.
· Responsible for performing evaluations and treatment of all facility wounds
· Organize information and communicate with physicians, managers, and supervisors on all issues related to wound care of our residents.
· Transcribe/ verify physician’s orders, coordinate treatments and other procedures as appropriate.
· Timely documents all pertinent resident information reflecting health status and response to care plan goals. Transcribes and notes orders.
· Supervises Certified Nursing Assistants which includes but is not limited to orienting, assigning, and evaluating.
