Position Overview
We are seeking an experienced and dedicated Regional Wound Care Nurse Practitioner to provide clinical oversight and specialized wound care services across multiple skilled nursing facilities. The ideal candidate will be passionate about geriatric care, have extensive wound management expertise, and thrive in a collaborative healthcare environment.
Key Responsibilities
- Assess, diagnose, and develop treatment plans for residents with acute and chronic wounds
- Provide direct wound care services including debridement, dressing selection, and specialized treatments
- Develop and implement facility-wide wound prevention protocols
- Conduct regular rounds across multiple facilities to monitor wound healing progress
- Collaborate with interdisciplinary teams including physicians, nurses, physical therapists, and dietitians
- Provide education and training to nursing staff on wound care best practices
- Maintain accurate and comprehensive documentation of assessments and interventions
- Track and analyze wound care outcomes to continuously improve quality of care
- Participate in care planning meetings for complex cases
- Ensure compliance with regulatory requirements regarding skin integrity and wound management
Qualifications
- Current state licensure as a Nurse Practitioner
- Master's degree in Nursing or higher
- Board certification as a Wound Care Certified (WCC), Certified Wound Specialist (CWS), or Certified Wound, Ostomy, and Continence Nurse (CWOCN)
- Minimum of 3 years of experience in wound care management
- Experience in long-term care or skilled nursing settings preferred
- Strong understanding of geriatric care principles
- Excellent clinical assessment and decision-making skills
- Ability to travel between multiple facilities
- Strong communication and leadership abilities
- Knowledge of Medicare/Medicaid regulations related to wound care
