Essential Duties & Responsibilities: The Women’s Intervention Specialist will provide screening and assessment activities for pre-natal, pre and interconceptional women for the purpose of identifying specific risk factors in eligible women of childbearing ages 15-44.  The WIS will also recruit and enroll the men involved with Magnolia Project women into case management services.

 

Specific Activities:

Screening & Assessment

·         Screen, and determine eligibility and immediately crisis needs of all referrals submitted to the Magnolia Project case management team for services.

·         Recruit, screen and determine eligibility for men involved with Magnolia Project women.

·         Conduct baseline assessment utilizing approved screening tools that are designed to assess family planning, social history, medical history (STD/HIV prevention), substance abuse and mental health, identification of strengths and limitations in participant’s life.

·         Complete all screening and assessment tools as defined in the project protocols.

·         Assist in determining the level of care necessary for each participant in the development of a comprehensive care plan.

·         Link ineligible women to other appropriate services providers within the community to include substance abuse treatment.

·         Function as an integral part of the team in enhancing productivity and ensuring progress towards recommended goals of each individual participant.

·         Meet participants in their homes, office, treatment centers or other community sites during traditional and non-traditional hours.

·         Provide written feedback to all referring persons/ agencies once the referral is assessed.

·         Enter and maintain participant data into the database daily to include, but not limited to progress notes, daily itineraries and travel.

·         Utilize the expertise of the nursing consultant within the case management team.

·         Maintain participant confidentiality and secure all documentation at all times.

·         Complete and submit all reports to the team lead as required.

·         Participate in scheduled training with the evaluators and maintain open communication.

·         Attend team meetings, case conference, and training as requested.

Other:

All All other duties as assigned by the Team Lead or Magnolia Project Director.



Education and/or Work Experience Requirements:

·         Bachelor degree preferred with at least 2 years professional experience in social services.

·         Will substitute for degree at least 5 years’ experience in social services to include at least 3 years in direct services with high risk women.

 

Additional Eligibility Qualifications:

·         Knowledge of maternal and child health, HIV/AIDS and substance abuse.

·         Ability to work independently

·         Ability to coordinate a multi-agency team and work effectively with diverse organizations and diverse communities

·         Knowledge of diverse communities

·         Develop quality assurance standards

·         Knowledge of data analysis and public health statistics

·         Ability to work independently and to carry out assignments to completion within parameters of instructions given prescribed routines, and standard accepted practices.

·         Must be able to communicate effectively and be a team player at the Coalition.

·         Must be able to commit to the Coalition vision and be passionate about its mission.

·         Requires reliable transportation as travel is required, valid driver’s license, good driving record and automobile insurance.

Physical Requirements:

·         Ability to safely and successfully perform the essential job functions consistent with the ADA, and other federal, state and local standards.

·         Must be able to travel to alternate locations.

·         Required to sit, stand, walk, speak and hear while performing the duties of this job, regularly. The position requires extensive computer use so the employee must have sufficient hand dexterity to use a computer keyboard and be capable of reading a computer screen

·         Lifting/carrying up to 25 pounds.

This position is dependent upon the continuation of funding. As such this position may terminate when the funding is no longer available.
