Job Title: Transition Nurse RN LPN

Position: Full-Time

Schedule: 10am – 6pm

Location: Cape Cod, Massachusetts

At Broad Reach Healthcare, we are seeking a Transition Nurse to join our dynamic team in the unique and picturesque setting of Cape Cod, Massachusetts. This position offers an exciting opportunity to coordinate patient admissions & discharges, and play a pivotal role in patient care.

Responsibilities:

·         Complete & monitor all paperwork for admissions and discharges.

·         Be the Face of our Nursing Team: Accompany patients to room upon admission.

·         Collaborate with our team of healthcare professionals to evaluate individualized care plans and communicate care plan information to staff members.

·         Assure continuity of care across all shifts.

·         Respond to emergency situations and provide direct care when needed.

·         Provide oversight of Therapeutic Dining group at lunch and dinner.

·         Assure staff adhere to infection control practices.

Qualifications:

·         Registered Nurse (RN) or Licensed Practical Nurse (LPN) Degree in Nursing required.

·         Current Massachusetts RN or LPN license in good standing.

·         Strong clinical expertise and knowledge of healthcare best practices.

·         Excellent communication and interpersonal skills.

·         Ability to prioritize and make sound decisions in a fast-paced environment.

·         Commitment to providing high-quality, patient-centered care.
