Transition Nurse
Position: Transition Nurse
Schedule: 10:00 AM – 6:00 PM Full-Time
Hourly Rate: $30 – $40/hour (based on experience)
We’re seeking a Transition Nurse to support our nursing leadership team and admissions department in delivering exceptional patient experiences from the moment a resident arrives.
Position Summary:
The Transition Nurse plays a critical role in coordinating the clinical and logistical aspects of both admissions and discharges across the facility. This role combines hands-on nursing with care coordination, documentation accuracy, interdisciplinary communication, and a two-day-per-week partnership with the Admissions Department to screen patients before admission. This ensures each resident admitted is clinically appropriate and prepared for a safe, welcoming transition.
You’ll provide continuity of care, serve as a clinical liaison between departments, and support staff in meeting regulatory, quality, and documentation standards. You may float between units as needed and serve as a clinical resource for complex care issues.
Key Responsibilities:
- Pre-Admission
Support (2 days/week):
Collaborate directly with the Admissions Department to review clinical documentation, assess medical appropriateness, and flag any care complexities prior to admission. Provide clinical input to help ensure safe, timely, and appropriate admissions. - Admissions
Coordination:
Accompany new patients to their rooms, initiate admission assessments, enter orders into the EMR, reconcile medications and treatments with a second nurse, and ensure consent forms and required documents are signed and filed properly. - Discharge
Planning:
Prepare and review discharge paperwork (including next-day discharges), communicate instructions clearly to staff and residents, and verify physician and family notifications are completed. - Clinical
Documentation & Oversight:
Ensure care plans and the KARDEX are accurate and up-to-date. Maintain awareness of patient records for regulatory compliance and audit readiness. Support entry of diagnostic orders (e.g., INR/Coumadin labs, x-rays, lab slips). - Direct Patient
Care & Staff Support:
Provide treatments, IV therapy, and INR testing as needed. Support unit staff in resolving clinical challenges and assist with therapeutic dining programs to ensure safety and adequate support during meals. - Communication
& Continuity:
Maintain effective communication with all shifts, unit managers, physicians, and support departments to facilitate care continuity and address clinical needs in real-time.
Qualifications:
- Licensure: Active RN or LPN license in the State of Massachusetts
- Experience: Minimum of 2 years in a skilled nursing, subacute, or transitional care setting
- Skills: Strong clinical judgement, EMR proficiency, clear written/verbal communication, and the ability to support and educate other staff
- Ability to multitask and adapt to shifting priorities in a fast-paced care environment
