MES provides housing case management services to underserved individuals and families by offering wrap-around services leading to housing stability and self-sufficiency. Our programs are designed to help residents with chronic homelessness transition into stable and permanent housing
 


Position Summary:

The Team Lead – with guidance from the Director of Case Management Services – is responsible for the initiation of PSH case management services program. The Team Lead is responsible for ensuring each participant has the opportunity to engage fully in relevant and appropriate vocational, social, academic and independent living activities (as deemed appropriate). The Team Lead provides day to day direct assistance to the case management staff, under the direction of the Program Manager and Director of Case Management Services. The Team Lead assists with the planning and monitoring of the program activities and direct case management services. The Team Lead will assist the Program Manager in leading day to day services for the PSH clients and ensure compliance with DHS standards as it pertains to client services and deliverables.

Essential Functions:

· Assist in the implementation of the comprehensive quality assurance system which is consistent with the overall goals and objectives

· Co-facilitate meetings with case management staff at minimum monthly to ensure staff effectiveness in service delivery

· Maintain a small caseload of PSH clients

· Assist in establishing productive collaborative partnerships/relationships with community members such that positive communication between the clients and home/community is routine

· Advocate for clients with referral and service agencies

· Complete monthly case management reports of clients and maintain current documentation of records

· Actively participate in MES and DHS training

· Monitor and manage client grievance calls and alert Program Manager, as needed

· Monitor client arrears for the assigned PSH Team

· Assist in client engagement strategies, including conducting home visits with new case managers and shadowing case managers in the field.

· Conduct field visits for current clients, as needed, to discuss current goals and services per the client’s ISP. Submit documentation chronicling Field Visit services, including case manager evaluation and client assistance.

· Conduct weekly client phone calls on behalf of the agency to ensure proper implementation of services by case managers and document contact on the Leadership Interaction Worksheet

· Work one-on-one with new Case Management hires in their 3-month probationary period, including but not limited to, shadowing home visits, conducting supervision, providing guidance and monitoring paperwork submissions

· Upload ISP and BPS signature pages into the QuickBase system and maintain records of the compliance from CMs

· Support the Program Manager on office days

· Conduct New Hire check-in meetings

· Perform other duties as assigned

Minimum Qualifications:

· Bachelor’s Degree in social work, psychology, sociology, and counseling or other related social service/science disciplines

· Six (6) years of case management experience and a high school diploma, GED, or other certification may replace education requirements

· Licensed Social Worker or a Licensed Professional Counselor in DC (Preferred)

· Minimum of four (4) years’ experience in the case management field

· Must have excellent motivation, communication, writing, organization and computer skills

· Must be able to support the company vision

· Successfully pass Police, Child Registry and Federal Background Clearance Checks

Working Conditions:

This is a full-time position, Monday through Friday, 9:00 a.m. to 5:00 p.m. Occasional evening and weekend work may be required as job responsibilities demand.

Physical Requirements

While performing the duties of this job, the employee will frequently sit, stand, walk, drive, and reach—prolonged periods of driving, walking, and possible meetings.
