Tailored Care Management (TCM) Supervisor

Pathways to Life, Inc. is seeking an experienced and dedicated Tailored Care Management Supervisor to lead our Care Management team in providing high-quality, person-centered services. This role is ideal for a passionate leader with a strong background in behavioral health, case management, and North Carolina Medicaid standards.
The ideal candidate will have exceptional leadership skills, a collaborative mindset, and a commitment to ensuring individuals with complex needs receive integrated care across behavioral health, physical health, and social support systems.
Position SummaryThe TCM Supervisor is responsible for overseeing the delivery of high-quality Tailored Care Management services in accordance with North Carolina Medicaid standards and agency policies. This role provides direct supervision to Care Managers, ensures regulatory compliance, and supports the effective coordination of services for individuals with complex needs. The Supervisor plays a key role in maintaining program quality, fiscal responsibility, and staff development.
Core ResponsibilitiesStaff Supervision & Evaluation-
Conducts performance evaluations and provides regular coaching, feedback, and guidance.

Manages team scheduling to ensure adequate staffing coverage.

Participates in the recruitment, hiring, and onboarding of Care Managers.

Develops and implements staff training plans to strengthen competencies and address performance gaps.

Monitors staff compliance with program standards, including timely and accurate documentation for reporting and billing.

Provides case consultation and clinical oversight, reviewing and approving client assessments and care plans.

Oversees day-to-day operations to ensure effective and efficient delivery of TCM services.

Coordinates team activities to meet service goals and client needs.

Reviews client records and reports to ensure compliance with agency, county, state, and CARF standards.

Monitors preparation, maintenance, and accuracy of client documentation.

Assists Program Manager with fiscal oversight to support cost-effective program operations.

Provides leadership and direction for new program initiatives and enhancements.

Assists in setting and monitoring service provision goals for the TCM team.

Promotes continuous improvement and alignment with agency, county, and state requirements.
North Carolina’s Tailored Care Management (TCM) services operate under the Tailored Care Management model launched December 1, 2022. The TCM Supervisor ensures that Care Managers deliver integrated, whole-person care that addresses:

Physical and behavioral health needs

Intellectual and developmental disabilities (I/DD)

Traumatic brain injuries (TBI)

Pharmacy services
-
Long-term services and supports (LTSS)
-
Social determinants of health (housing, food, transportation, etc.)

A license, provisional license, certificate, registration, or permit issued by the governing board regulating a human service profession, such as:

Licensed Clinical Social Worker (LCSW)
-
Licensed Marriage and Family Therapist (LMFT)
-
Licensed Clinical Addiction Specialist (LCAS)
-
Licensed Clinical Mental Health Counselor (LCMHC)
-
Licensed Psychological Associate (LPA)
-
Registered Nurse (RN) licensed by the North Carolina Board of Nursing

Experience in behavioral health, case management, or related services.

Knowledge of NC Medicaid rules and regulations, Tailored Care Management, and local community resources.

Strong written, verbal, and interpersonal communication skills.

Demonstrated ability to lead, train, and supervise staff effectively.

Commitment to compliance, quality assurance, and person-centered care.

Prior supervisory or management experience in behavioral health or case management.

Familiarity with CARF accreditation standards.

Experience working with individuals with complex behavioral health, I/DD, or TBI needs.

Proficiency in electronic health records (EHR) and Medicaid billing documentation.

Standard office environment with some travel to community sites, client homes, and partner agencies as needed.

Full-time, exempt position with flexibility for program needs.
