Cleveland Health Care Center is seeking a compassionate and dedicated Speech Language Pathologist (SLP) to join our in-house therapy team. What You’ll Do:-
Evaluate and assess residents for speech, language, cognitive, and swallowing disorders

Develop individualized treatment plans and provide skilled therapy services

Educate residents, caregivers, and staff to support continued success outside therapy

Utilize AAC systems and support patients with complex communication needs

Collaborate with interdisciplinary teams for diagnostic procedures and care planning

Document progress and outcomes accurately using our electronic documentation system

Participate in caregiver training and support restorative nursing programs

Uphold resident rights, maintain confidentiality, and comply with all regulatory standards

In-house therapy model – No contract rotation

Active license as a Speech-Language Pathologist in Texas

CCC in Speech-Language Pathology (Required)

Experience in geriatric or skilled nursing preferred
