Cleveland Health Care Center is seeking a compassionate and dedicated Speech Language Pathologist (SLP) to join our in-house therapy team. Located just 45 miles north of Houston, our 142-bed skilled nursing facility has proudly served the Cleveland, TX community for over 50 years. As the most established health care provider in Liberty County, we’re known for delivering high-quality, resident-focused care in a welcoming environment.

• Evaluate and assess residents for speech, language, cognitive, and swallowing disorders

• Develop individualized treatment plans and provide skilled therapy services

• Educate residents, caregivers, and staff to support continued success outside therapy

• Utilize AAC systems and support patients with complex communication needs

• Collaborate with interdisciplinary teams for diagnostic procedures and care planning

• Document progress and outcomes accurately using our electronic documentation system

• Participate in caregiver training and support restorative nursing programs

• Uphold resident rights, maintain confidentiality, and comply with all regulatory standards

• In-house therapy model – No contract rotation

• Active license as a Speech-Language Pathologist in Texas

• CCC in Speech-Language Pathology (Required)

• Experience in geriatric or skilled nursing preferred
