Job Description:

We are looking for a caring and dedicated Licensed Social Worker to join our team. You will be responsible for assessing, planning, and coordinating social services for our residents and their families, ensuring they receive the best possible care and support.

Key Responsibilities:

As a Licensed Social Worker at Glendale Place Nursing & Rehab Center, you will conduct assessments, develop and adjust care plans, provide counseling and support to residents and families, coordinate with interdisciplinary teams, advocate for residents' rights, connect them with community resources, and maintain accurate records while participating in quality improvement initiatives.

Qualifications:

- State licensure as a social worker.

- At least two years of experience in a skilled nursing facility or geriatric setting.

- Excellent communication and interpersonal skills.

- Strong organizational and time management skills.

- Ability to work effectively as part of an interdisciplinary team.

- Knowledge of Medicare/Medicaid regulations and documentation requirements.

- Proficiency with Microsoft Office and electronic medical record systems.
