About the Role
We are seeking a licensed, compassionate, and experienced Social Worker to join our interdisciplinary team. This role is essential in supporting residents and families through every stage of their stay, from admission to discharge, while ensuring the highest level of dignity, advocacy, and care coordination.
What You’ll Do
Complete comprehensive psychosocial assessments upon admission and throughout stay
Provide emotional support and counseling to residents and families
Lead and coordinate discharge planning with the interdisciplinary team
Connect residents with community resources and post-discharge services
Participate in care plan and IDT meetings
Advocate for resident rights, dignity, and quality of life
Address and resolve resident/family concerns in a timely, professional manner
Maintain accurate and compliant documentation
Support admissions and communication with referral sources as needed
What We’re Looking For
Active Texas Social Worker license (LSW or LMSW) – REQUIRED
Bachelor’s Degree in Social Work (BSW) required; MSW preferred
Experience in skilled nursing, long-term care, or healthcare strongly preferred
Knowledge of discharge planning, care coordination, and community resources
Strong communication, organization, and problem-solving skills
Ability to thrive in a fast-paced, team-oriented environment
