Director of Social Services

Transitions Healthcare Allens Cove is currently seeking a Full- Time Director of Social Services to join our team of talented caregivers.

Director of Social Services Responsibility Highlights include:

- Meeting with residents and/or responsible parties to review policies on advanced directives, living wills, DNR status, residents rights and discharge planning. Implement documents as appropriate with residents and/or their responsible parties. 

-Conduct and document social service evaluations including idenification of residents needs/issues and arrange for appropriate social support services. 

-Provide assistance to residents and/or their responsible parties in obtaining financial services, as appropriate. 

-Ensure that social servvices are coordinated with the other facility services.
