Location: Miami, FL

A Skilled Nursing Facility is seeking an experienced Social Services Director to oversee psychosocial services, discharge planning, and resident advocacy within the facility. This is a full-time, permanent leadership role requiring prior experience in a Skilled Nursing or long-term care setting.

The Social Services Director will ensure compliance with federal and state regulations while supporting residents and families through admissions, care planning, and discharge transitions.

Key Responsibilities

• Oversee all social services operations within the Skilled Nursing Facility
• Lead discharge planning processes and coordinate transitions with hospitals and community providers
• Ensure compliance with CMS and state long-term care regulations
• Participate in interdisciplinary care plan meetings and QAPI initiatives
• Address resident rights, grievances, and family concerns
• Maintain accurate, compliant documentation
• Collaborate with Admissions, Nursing, Therapy, and Administration
• Support survey readiness and regulatory processes

Required Qualifications

• Minimum 2+ years of experience as a Social Worker or Social Services Director in a Skilled Nursing Facility required
• Bachelor’s degree in Social Work, Human Services, or related field required (Master’s preferred)
• Strong understanding of CMS regulations and long-term care compliance
• Experience with Medicare, Medicaid, and discharge planning processes
• Proven ability to manage resident and family communication in a SNF environment

Hospital-only experience will not be considered.
