Director of Social Services

Heather Hill Healthcare Center is seeking a full time Director of Social Services. The Director of Social Services is a member of the interdisciplinary and management team of the nursing center. Social Services Director oversees the overall operation of the Resident and Family Services Department. Informs residents, representatives and staff of patient rights. Serves as center Ombudsman to ensure resident rights are honored. Builds and maintains good working relationships with residents, family members, community agencies and other employees. Works with residents and families towards adjustments to institutional living and the crises of illness, disability and death. Serves as the facilities “Grievance Officer” to ensure that all grievances are investigated and resolved in accordance to the facilities grievance policy. Coordinates the Customer Service/ Satisfaction Program at the facility. Participates in the MDS/ Care Planning process and documents accordingly. Documents resident progress and significant concerns/ events in medical records. Participates in the facilities Quality Assurance/ QAPI program. DIRECTOR OF SOCIAL SERVICES QUALIFICATIONS:

· Bachelor’s Degree in a Human Service Field REQUIRED, Master’s in social work preferable.

· Interpersonal skills necessary to communicate accurately and precisely to patients and family members

· Experience in a skilled nursing facility.
