Social Services Designee - 8a-4p, Part-Time

Summary

Under the direction of the Administrator / Social Worker:

  • Utilizing knowledge of the casework service process and employing associated techniques, [the Social Service Designee] interviews, counsels, assesses, and intervenes (as needed), in order to affect positive change in the elderly / disabled resident's emotional, behavioral, and psychosocial functioning
  • Accesses / coordinates appropriate treatment and services for resident psychosocial adjustment difficulties and effectively communicates with team members, residents, resident families, legal representatives, and community resources accordingly
  • Ensures that the corporate / care center social services policies and procedures for pre-admission, admission, advocacy, direct / indirect counseling, discharge, and community liaison services are in compliance and implemented in a timely and efficient manner

Qualifications / Requirements

  • High school diploma or equivalent with post-secondary education and/or work experience in a human services field preferred
  • Must be knowledgeable of community resources and support services
  • Per the Centers for Medicare and Medicaid Services (CMS), COVID-19 vaccination is required, with exceptions for medical and religious accommodations when reasonable

Additional Information

Job Type: Part-Time

Shift: 8a-4p, Flexible Scheduling; additional hours available if willing to cross-train for Full-Time status

Wage Range: $20.00/hr-$24.00/hr, Negotiable (DOE)
