Social Services Designee

Job Summary:

We are seeking a compassionate and organized Social Services Designee to join our team at Grand Traverse Pavilions. In this role, you will support our residents and their families by coordinating social services and advocating for their needs and rights within our facility.

Key Responsibilities:

• Coordinate and deliver social services to residents and families to enhance their healthcare experience.
• Conduct admission interviews, develop treatment plans, and maintain accurate and confidential records.
• Advocate for residents' rights and ensure compliance with state and federal guidelines.
• Facilitate discharge planning and connect residents with community resources.
• Provide emotional support and assist residents in adjusting to life in the facility.
• Collaborate with healthcare staff to address residents' psychosocial needs.

Qualifications:

• High school diploma or equivalent; completion of a state-approved Social Service Designee course preferred.
• Bachelor's degree in social work or related field is a plus.
• Minimum of one year of experience in a healthcare or social services setting.
• Excellent communication, organizational, and advocacy skills.
• Knowledge of state regulations for skilled nursing facilities.
