RN Case Manager

Vitality Living of Park Rapids is  seeking a RN Case Manager to support the community. This role works collaboratively with interdisciplinary teams to ensure the delivery of high-quality, resident-centered care in compliance with Minnesota Assisted Living Licensure requirements.

The RN Case Manager provides clinical leadership, oversight, education, and support to care teams while ensuring accurate assessments, care planning, and regulatory compliance across both locations.

Key Responsibilities

·       Conduct initial, routine, and change-in-status nursing assessments in accordance with state regulations

·       Develop, review, and maintain resident care plans and ensure services are delivered as assessed

·       Provide clinical leadership, guidance, education, and competency support to nursing and care staff

·       Collaborate with interdisciplinary teams to support resident safety, quality outcomes, and care coordination

·       Ensure documentation is accurate, complete, and submitted timely for reimbursement and regulatory purposes

·       Support ongoing compliance with Minnesota Assisted Living Licensure requirements

·       Serve as a clinical resource for leadership and staff at both Park Rapids and Sebeka communities

Qualifications

·       Active RN license in the state of Minnesota (required)

·       Experience in Assisted Living, Long-Term Care, or related healthcare setting preferred

·       Strong knowledge of assessments, care planning, and regulatory compliance

·       Ability to work independently while collaborating effectively with multiple teams

·       Strong organizational, communication, and leadership skills
