What We Do:

Our mission is to guide brain injury survivors, individuals with disabilities, and their families, by ‘bridging the gap’ between surviving and living lives of independence with dignity and hope. Our organization assists people to rebuild skills they were once able to do independently before terrible tragedy struck in the form of a car accident, stroke, assault, fall, or before they were struck with a debilitating disease.


Service Coordination Supervisor Responsibilities:

1.      Assure Department of Health and organizational deadlines and timelines are met by the Service Coordination Department.

2.      Assist Service Coordinators in problem-solving and crisis intervention and prevention.

3.      Facilitate monthly service coordination department team and individual supervisory meetings.

4.      Attend Department of Health Primary Contact and/or Provider meetings as required.

5.      Review service plan and ensures correction submission as necessary

6.      Orient new Service Coordinators and conducts mandatory annual training to service coordination team

7.      Conduct performance reviews and appraisals to service coordination team.

8.      Promote and market organization services via regular presentations, meetings, and contacts to potential referral sources and maintain positive working relationships to build alliances.

9.      Attend ongoing Management Team meetings and supervisory meetings with CEO.

10.   Reviews SC weekly billing for accuracy, approves and forwards to accounting office in a timely manner.

11.   Provide on-call assistance as requested by the CEO.

12.   Participate in conferences, workshops, and in-service training to enhance job skills and personnel growth and development.

13.   Adhere to agency policies and procedures.

14.   Perform other duties as assigned.

 Job Specifications:

Ø  Education: Must have a BA degree with one year experience providing service coordination for individuals with disabilities.  OR associate’s degree with three years’ experience providing service coordination for individuals with disabilities.

Ø  Experience: Knowledge of and accessibility to community resources, knowledge of TBI Waiver services and a minimum of 2 years’ supervisory experience. 

Ø  Skills: Effective problem-solving skills, ability to professionally represent and promote Bridges For Brain Injury, Inc., and ability to work with confidential information.  Interpersonal Skills: Excellent oral and written communication skills, strong interpersonal skills, ability to collaborate with other members of the Team and interact effectively with all levels of personnel.  Must be flexible and have good judgment, common sense, and be dependable.

Ø  Other: Valid NYS driver’s license and vehicle meets NYS operating standards.

Ø  Upon hire, candidates must present evidence of a negative PPD Skin test completed within the last 2 years (24 months) or have a new PPD Skin test completed during the first 30-days of hire.
