Qualifications:

Unencumbered licensure in the state of Texas for RN or LVN

Previous wound care nurse experience a plus

Able to lift 60-70 pounds

Able to work independently or with little oversight

Organized and Detailed

Able to use computer system (Point Click Care) to document 

Job Description:

Conduct thorough weekly skin assessment of all residents in the facility and document assessment in PCC

Obtain and transcribe physician orders for wound treatments

Perform treatments for all wounds and skin issues in the facility per physician orders and using proper infection control measures

Prepare a weekly skin report 

Document new skin issues in resident care plan and update as needed

Conduct weekly wound rounds with wound care physician and update orders as needed

Attend clinical meetings as required 

Nurse On Call rotation with clinical administrative team
