Vitality Living of New York Mills is seeking a part-time RN Case Manager.  The successful candidate will work cooperatively with the inter-disciplinary team to ensure the delivery of quality care to all residents based on the needs assessed and determined. The RN supports the clinical team with leadership, management, education, training and competencies, as appropriate. Ensure that initial, routine and change in status assessments are conducted as required, care plans and services are maintained and provided and submitted for reimbursement. The RN will support overall compliance at Vitality Living locations based on current Assisted Living Licensure requirements.

Required Skills/Abilities:

• Proficiency with systems and technology including electronic medical records, Microsoft 
• Must be able to multi-task and prioritize based on resident and staff need throughout the day 

Education and Experience:
• Must be licensed with the State of Minnesota as an RN and must maintain current and unencumbered licensure.
• At least 3 years’ experience in assisted living or senior care setting preferred
