Role: Works within the Health and Wellbeing Team Provides assistance to the resident in maintaining their highest level of functional independence and psychosocial wellness.
Responsibilities:
· Ensure compliance to Juniper Village’ philosophy and hallmarks as well as all written policies and procedures that govern the Health and Wellbeing Department.
· Ensures resident safety at all times.
· Embraces Culture Change and resident-centered philosophies
· Perform continuous, ongoing supervision of all Restorative team members, including but not limited to training, disciplining, counseling, motivating, and evaluating.
· Complete all assigned paperwork to meet current regulatory compliance standards.
· Ensures correct procedures when conducting functional maintenance programs, applying splints, pressure relieving devices, other positioning or range of motion equipment
· Complete restorative documentation with accuracy and thoroughness.
· Assist in the ongoing oversight of Wellness Associates: CNAs assigned to restorative tasks through training, mentoring and coaching.
· Suggest referrals for those who would benefit from therapies or functional maintenance programs
· Participate in care conferences of those residents who experience frequent falls, who are on a bowel and bladder program, who are Medicare A
· Educate staff regarding restorative programs such as exercise, range of motion, strengthening, life skills training, restorative dining, work therapy
· Create monthly schedule for Restorative Staff; create daily schedule to ensure resident restorative needs are met.
· Monitor Restorative Staff for correct procedures when conducting Functional Maintenance Plan (FMP’s), applying splints, pressure reliving devices, other positioning /range of motion equipment.
· Participate in “In Touch” Training. Act as mentor to all Restorative Associates.
· Annually evaluate performance of Restorative associates.
· Upon Admission, quarterly, and as needed, assess or sign off on Restorative Screens within 72 hours.
· Assess each resident who falls for restorative needs. Obtain fall incident reports and provide report at Quality of Life meetings each week.
· Assess each resident who has newly developed pressure areas for pressure relieving equipment.
· Prepare initial restorative care plans and update quarterly or with change in condition.
· Assist in the planning and implementation of the In-Touch Orientation and in-service training.
· Communicate with family members, visitors and volunteers in a customer service-oriented manner.
· Follow established safety regulations, including fire protection, infection control, bloodborne pathogens, hazardous waste and quality assurance procedures.
· Participates in meetings as required. Meetings to include weekly updates at quality of life meetings, QAPI meetings, care conferences
· Carry out QAPI roles and responsibilities as assigned.
· Follow established policies and procedures in support of QAPI efforts.
· Report all changes in the resident’s condition, and skin breakdown to the Director of Wellness or Neighborhood Wellness Coordinator
· Assist in the coordination of effective communication among Wellness Associates, residents and families Report all complaints, grievances and instances of abuse to the Neighborhood Wellness Coordinator or Director of Wellness.
· Report all occupational exposures to blood, body fluids, infectious materials and hazardous chemical to the Neighborhood Wellness Coordinator Director of Wellness.
· Participates in meetings as required. Meetings to include weekly updates at quality of life meetings, QAPI meetings, care conferences
Educational/Vocational Requirements:
· Must be a CNA or LPN
· Experience in long-term care setting
· Knowledge of the physiology and psychology of the older adult.
· Successful completion of a CPR and First Aid course.
