The Service Coordinator is responsible for enabling residents of Fowler Christian Apartments to continue to live independently at the highest possible level of spiritual, emotional and physical functioning. The Service coordinator is responsible for ensuring  residents of our Senior living Affordable Community are aware of and connected to the necessary and desired support service available.

Some key responsibilities include:

1.      Maintain and provide oversight and awareness of residents physical and mental condition.

2.      Research, develop, and maintain a network of service providers and eligibility requirements. Establish and maintain a service directory of local community resources.

3.      Provide general case management for residents and provide appropriate referrals to service providers/community agencies as needed

4.      Identify and assess residents who are at risk. Recognize when residents need higher level of care to maintain or improve their safety and health. Assist residents and families in locating and assessing an appropriate facility.

5.      Maintain an individual file for each resident documenting all incidents related to the residents condition/situation.

6.      Respond to activated resident emergency calls during regular office hours.

7.      Conduct/assist as needed annual apartment inspections with maintenance to assess residents living environment.

8.      Maintain and supervise the planning, scheduling and performance of activities for the residents as needed. Provide social opportunities that assist residents in building informal support networks between residents, family and friends.

9.      Act as ex-officio member of resident council; attend meetings, and accept reports from wing- reps and monitors.

10.  Visit with and follow-up on residents in the hospital. Coordinate hospital reports with staff, monitors and wing-reps.

11.  Respond to inquiries concerning eligibility for HUD and HUD/STAR+ paired Assisted Living facility.

12.  Maintain current knowledge of HUD, STAR+ and Assisted Living regulations.

13.  Educated or schedule training on service availability, application procedures, residents rights, aging in place, sensitivity to the aging process and other topics as appropriate to staff, families and the community.

14.  Organization, implementation and facilitation of residents volunteer activities within the community; coordinate and assist with outside volunteer activities and groups.

15.  Perform other duties as assigned.

Qualifications/Requirements:

1.      Graduate of an accredited university with a degree in Social Work, Gerontology, Psychology, Sociology, Counseling or related field experience.

2.      Verifiable experience in a social service delivery system; minimum of 10 years.

3.      Experience in working with the elderly and/or disabled; knowledge of the aging process and issues.

4.      Experience in the successful development and execution of a senior activity program.

5.      Must possess strong interpersonal skill and the capacity to relate to a variety of people and personalities.

6.      Must possess strong analytic skills and be able to form comprehensive solutions to the situation you have analyzed.

7.      Must posses strong organizational, record keeping, and time-management skills, and be able to work independently and as part of a team, with minimal supervision.

8.      Must have working knowledge of personal computers and ability to learn various programs/applications.

9.      Must attend weekly staff meetings, providing updated information from the department.

10.  Possess a valid drivers license and able to operate a motor vehicle.

11.  Ability to work flexible hours and be on call.

12.  Ability to comply with residents rights, all regulatory requirements, and all company policies and procedures as stated now and in the future.

13..  Must be able to read, write, speak and understand the English language.

14.  Must possess the ability to make independent decisions with circumstances warrant such action.

15.  Must be a supportive team member, contribute to and be an example of team work and team concept.

16.  Must possess leadership ability and willingness to work harmoniously with other personnel.

17.  Must have patience, tack, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently function.

18. Bilingual is a plus
