Position Title: Registered Nurse (RN), Licensed Dementia Housing

Department: Long Term Care, Brentwood House/Sluggett House

Location: Brentwood Bay, BC

Days/Hours of Work: Monday –Friday 8:30am –4:30pm

JOB SUMMARY:

Reporting to the Long Term Care Manager, the RN is responsible for Quality Nursing care at Brentwood House and Sluggett House. The RN performs assessments and, in conjunction with the Manager and LPN, assists in developing and coordinating the implementation of individualized care plans and maintains dignity and respect for residents’ rights. The RN will support a Social Model of individualized care in collaborating with residents, family, and a multi-disciplinary team. The RN operates within an environment that encourages resident and staff participation and promotes independence.

The RN operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice, as outlined by the British Columbia College of Nursing Professionals. The RN will also operate according to Beacon Community Services’ policies, standards, and site-specific protocols.

DUTIES AND RESPONSIBILITIES:

  • Collaborates and over sees and reviews resident’s Nursing care at Brentwood House/Sluggett House, initial and ongoing assessments, including actual and potential strengths and limitations of cognitive, physiological, psychological, socio-cultural, and spiritual needs.
  • Works alongside care staff 3 days per week at Sluggett House and 2 days per week at Brentwood House.
  • Oversees and assists Full scope LPNs develop and monitor each resident’s individualized care plan using RAI MDS 2.0 data, and admission and Initial Care Conference data. Explains plan of care to residents and their families.
  • Reviews, documents and updates all relevant information related to resident care and procedures to achieve resident safety and comfort.
  • Develops RN and LPN nursing procedures as necessary.
  • Participates in admissions, discharges, and transfers of residents according to site operating policies and procedures.
  • Encourages, educates and supports LPN/ and care staff. Provides input into care staff/LPN performance and evaluations.
  • Assists in the orientation and continuing education of new staff, students and other members of the Health Care Team.
  • Participates in the continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings and committees.
  • Liaises with Residential Case Manager, Geriatric Specialty Services, Pharmacists, Nurse Practitioners, Physicians and Families.
  • Demonstrates evidence-based practice to meet the needs of the resident/client. Monitors and documents relevant observations, assessments and interventions including resident and family teaching and evaluation of care.
  • Uses designated protocols to ensure unusual or serious incidents are managed and reported promptly.
  • Assists with staffing coordination
  • Assists and plans care conferences and Medication reviews
  • Provides backup for the Long Term Care Manager as required.
  • Performs other related duties as required.

QUALIFICATIONS:

  • RN Certification
  • Current Registration BCCNP
  • Experience in geriatrics and dementia care
  • Proficient in RAI MDS 2.0
  • Current First Aid including CPR

  • Ability to communicate effectively, both verbally and in writing, with residents, families, co-workers, members of the multi-disciplinary team and the public
  • Ability to deal with others effectively
  • Ability to organize work and implement a plan of care
  • Knowledge and competencies to work at full scope of practice
  • Basic computer skill competency
  • Ability to operate related equipment
  • Physical ability to carry out the duties of the position
