(RN) Registered Nurse- Home Health

Job Type: Part-Time/Per Diem/Full-Time (Hours Vary by Case)

Job Description:

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our home health care team in Cleveland, Ohio. As a Home Health RN, you will provide skilled nursing care to patients in the comfort of their homes, helping them maintain independence and improve their quality of life.

Responsibilities:

• Conduct in-home patient assessments and develop individualized care plans

• Administer medications, treatments, and injections as prescribed

• Monitor and document patient condition and progress

• Coordinate with physicians, therapists, and family members to ensure comprehensive care

• Educate patients and families on disease processes, medications, and self-care techniques

• Adhere to all clinical and regulatory standards and company policies

Requirements:

• Active RN license in the state of Ohio

• Current CPR certification

• At least 1 year of nursing experience (home health preferred)

• Strong clinical and communication skills

• Reliable transportation and valid driver’s license
