Position Summary

The Registered Dietitian (RD) at Health Care Partners of South Carolina is a clinical expert responsible for providing high-quality Medical Nutrition Therapy (MNT) to a diverse, underserved patient population. This role focuses on the prevention and management of chronic conditions, specifically obesity, metabolic diseases, and diabetes, by developing individualized nutrition plans that respect patients' cultural preferences and economic realities. Operating within an integrated, multidisciplinary care team, the RD empowers patients to achieve health goals through personalized counseling and evidence-based interventions.

Essential Duties and Responsibilities

  • Comprehensive Nutrition Assessment: Evaluate medical histories, lab results, and dietary habits to identify nutrition-related risks.
  • Medical Nutrition Therapy (MNT): Develop personalized care plans to manage diabetes (Type 1, 2, and Gestational), obesity, and metabolic disorders (hypertension, dyslipidemia).
  • Culturally Competent Counseling: Provide one-on-one and group counseling sensitive to the cultural and socioeconomic backgrounds of the community.
  • Chronic Disease Management: Educate patients on self-management techniques, including blood glucose monitoring and carbohydrate counting.
  • Interdisciplinary Collaboration: Coordinate with providers and social workers to ensure nutritional goals align with overall medical treatment.
  • Clinical Documentation: Maintain accurate records in the Electronic Health Record (EHR) using the ADIME (Assessment, Diagnosis, Intervention, Monitoring, and Evaluation) format.

Additional Responsibilities

  • Perform other duties as assigned to support organizational goals and compliance requirements

Qualifications

  • Education: Bachelor’s or Master’s degree in Dietetics or Nutrition from an accredited institution.
  • Credentials: Current registration with the Commission on Dietetic Registration (CDR) and valid state licensure.
  • Experience: Minimum 2 years of clinical experience
  • Certifications: Certified Diabetes Care and Education Specialist (CDCES) preferred.
  • Language: Proficiency in a second language (e.g., Spanish) is highly preferred.

Physical Requirements & Technical Proficiency

  • Physical: Ability to stand/walk for extended periods; manual dexterity for Nutrition-Focused Physical Exams (NFPE); ability to lift up to 15 lbs.
  • Technical: Proficiency in Electronic Health Records (EHR) (e.g., Epic, Athenahealth) and Microsoft Office Suite. Familiarity with telehealth platforms.
