Job Title:        Quality of Life CNA

Department: Activities

Reports to:     Activities Director

 

General Purpose:

To improve the quality of life for residents through building valuable relationships to enhance a personalized balance of planned activities and meaningful leisure time to fulfill the dimensions of wellness: physical, spiritual, intellectual, emotional, and social.

 

Qualifications:


·         Must maintain current CNA certification.

·         Must have the ability to relate positively and favorably to residents, their families, and work cooperatively with other staff members.

·         Must have the ability to read, write, and follow oral and written directions at a level necessary to accomplish this job.

·         Must have a genuine enthusiasm and sense of humor with ability to laugh at successes and failures.

·         Must be a self-motivated, creative and positive personality, who demonstrates willingness to learn new ideas and skills.

·         Provide person-centered care that emphasizes the residents’ comfort, independence and personal needs and preferences.

·         Must possess patience, assertiveness and be a conscientious person.

·         Must present a professional appearance.

·         Must be able to lift 60-70 lbs. frequently.

·         Must be able to stand and/or walk throughout most of scheduled shift.

·         Must be flexible in working hours so that weekend and evening events may be provided.

·         Must be able to meet all local health regulations and pass pre/post-employment physical exam if required. This requirement also includes drug screening, criminal background investigation and reference inquiry.

·         Must comply with attendance policy and established schedules, be flexible, and make necessary accommodations for the needs of residents and family members.

·         Must attend in-service/education programs as required to learn new procedures and develop skills to meet regulatory compliance.

·         Must agree not to disclose resident protected health information and report suspected or known HIPPA violations to the Administrator.

 

Duties and Responsibilities:

1)      Provide/facilitate therapeutic activities which meet the resident’s interests, while providing an atmosphere of fun and enjoyment.

2)      Maintain good relations with residents, families, and staff members.

3)      Be aware of resident’s rights.

4)      Report to Activity Director any changes in resident’s activity interests and/or needs.

5)      Assist in cleaning/maintaining all equipment used in Activities to meet company/state standards.

6)      Be aware of and report any problems with Activity equipment to Activity Director and Maintenance.

7)      Report any supplies needed for an activity that may be low/need replaced.

8)      Attend center meetings as required.

9)      Attend the monthly QOL staff meeting.

10)  Assist and accompany residents on off-facility outings as scheduled.

11)  Provides assistance with wheelchair transportation for residents to activity programs of choice.

12)  Be aware of and follow all state, federal, company and center policies and procedures.

13)  Keep Activity Director informed of any necessary changes in activities, problems and possible solutions.

14)  All other duties as assigned.

 

Physical and Sensory Requirements: Walking, sitting, standing, reaching, stooping, bending, lifting, grasping, pushing and pulling, and fine-hand coordination. Ability to hear and respond to overhead pages. Ability to communicate with residents, families, personnel, vendors, and consultants. Ability to apply training and in-service education provided. Must present a neat, clean, professional appearance and demonstrate a positive approach with employees and residents.
