A full-time exempt status position for the purpose of assessing, authorizing, supervising, and delivering care to adults with serious mental illness. The position also administers, manages, and operates the Community Support Services (CSS) program by supervising and directing a team of employees. In addition, the PC ensures that high quality care is delivered in accordance with Agency policies, procedures, and practices.

Responsibilities:

  • Performs a comprehensive rehabilitation needs assessment.
  • Contributes to the development, implementation, monitoring, and updating of the individualized rehabilitation plan.
  • Perform therapeutic rehabilitative skill development.
  • Offers services appropriate to meet the consumers’ needs.
  • Perform crisis intervention and 24 on-call support to employees and consumers for crisis intervention and emergency services linkages.
  • Submits care recommendations to the State Interim Management Entity (IME) to secure authorizations for care.
  • Coordinates, manages, supervises, and delivers all services and in conjunction with the CSS team including assessment, evaluation, rehabilitative planning, case conferences, referrals for adjunctive services, family meetings, group work, individual counseling, consumer advocacy, liaison with other community agencies, discharges, and any other services outlined or defined in the CSS regulations.
  • Participates in program development, implementation of new initiatives, and marketing/community engagement activities.
  • Provides support, clinical supervision, and direction to all program employees including orientation, training, performance evaluation, and professional development.
  • Monitors, as needed, consumer self-administration of prescribed medications as per agency procedures.
  • Assists consumers with accessing and engaging in community based wellness and recovery services.
  • Protects client rights and responsibilities.
  • Ensures services provided adhere to local, state, federal, and accreditation standards.
  • Follows Agency financial procedures.
  • Transports consumers in Agency vehicles in a safe, cautious, and responsible manner.
  • Documents service delivery accurately and timely while ensuring consumer confidentiality.
  • Organizes, attends, and participates in team, committee, supervision, and training meetings.
  • Coordinates with other providers to maximize services integration and consumer opportunity.
  • Assumes other duties and responsibilities as assigned by supervisors and senior managers.

Qualifications:

  • Must be either a NJ Licensed Clinical Social Worker (LCSW), NJ Licensed Professional Counselor (LPC), or NJ Registered Nurse.
  • In addition to a Master’s degree, licensed marriage and family therapists must have one year experience in a community behavioral health setting or be certified as a psychiatric rehabilitation practitioner (in lieu of the one year of experience in community behavioral health setting).
  • Experience providing services to individuals with serious mental illness.
  • Knowledge of mental health rehabilitative principles.
  • Valid NJ driver’s license, with a good driving record.
