Position Summary:
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our team on a PRN basis. In this role, you will provide high-quality nursing care in a homelike setting, supporting our residents’ physical and emotional well-being. You will work closely with a dedicated team of caregivers to ensure continuity of care and uphold the dignity and independence of every resident.

Responsibilities:

• Deliver hands-on nursing care in accordance with the resident's care plan

• Administer medications and treatments as prescribed

• Monitor residents for changes in condition and report findings to the RN or physician

• Document all care accurately and timely in the electronic health record

• Collaborate with caregivers, residents, and families to create a person-centered care environment

• Participate in shift change reports and team meetings

• Foster a supportive, home-like atmosphere through respectful and compassionate interactions

Qualifications:

• Current and valid Licensed Practical Nurse (LPN) license in the state of [State]

• CPR certification (required)

• Experience in long-term care, geriatric nursing, or similar setting preferred

• Strong interpersonal and communication skills

• Ability to work independently and as part of a team

• Flexible and available to work varying shifts as needed (including weekends or holidays)
