The Physician is responsible for providing comprehensive, high-quality medical care to patients in accordance with the Connecticut Department of Public Health and State Medical Board standards. The physician performs patient assessments, diagnoses, develops treatment plans, and manages acute and chronic health conditions while promoting preventative care and overall wellness.
This role requires clinical excellence, strong communication skills, and a patient-centered approach to care. The ideal candidate demonstrates leadership, compassion, and a commitment to improving health outcomes in the community.
Key Responsibilities-
Conduct comprehensive medical evaluations, including history taking, physical examinations, and diagnostic testing.

Diagnose and manage acute, chronic, and preventive health conditions in accordance with clinical guidelines.

Develop and implement individualized treatment plans tailored to patient needs.

Prescribe and manage medications and therapies responsibly and in compliance with DEA and state regulations.

Order and interpret laboratory, imaging, and diagnostic test results.

Provide patient education on disease prevention, lifestyle modification, and treatment adherence.

Collaborate with nurses, specialists, and other healthcare professionals to coordinate care.

Maintain accurate, timely electronic health records (EHR) documentation.

Ensure compliance with HIPAA, OSHA, CMS, and Connecticut Department of Public Health standards.

Participate in quality improvement, peer review, and continuing medical education (CME) activities.

Adhere to evidence-based practices and ethical standards of care.

Active and unrestricted Connecticut Medical License (MD or DO).

Board Certification or Board Eligibility in [Specify Specialty—e.g., Internal Medicine, Family Medicine, etc.].

Graduation from an accredited medical school and completion of an approved residency program.

Current DEA registration and Connecticut Controlled Substance Registration (CSP).

Current BLS/ACLS certification.

Minimum 2 years of clinical experience preferred (newly licensed physicians will be considered).

Strong diagnostic, clinical, and interpersonal skills.

Proficiency in electronic health records (EHR) systems.

Must be available at least 3 days per week, with flexibility for extended hours or weekends if required.

Experience in primary care, urgent care, home health, or long-term care settings.

Commitment to patient-centered, culturally competent care.

Leadership or supervisory experience in a clinical setting.

Familiarity with value-based care and quality reporting programs.
