Physician (MD/DO) – Internal Medicine or Family Practice – Skilled Nursing Facilities / Nursing Homes

Location: Orlando Area


Overview:

A physician-led organization is seeking a Board-Certified/Board-Eligible Physician (MD/DO) in Internal Medicine or Family Practice to provide care within Skilled Nursing Facilities (SNFs) and nursing homes. The focus of this role is on improving patient outcomes and reducing avoidable hospitalizations. This is a unique opportunity for a physician who enjoys clinical practice but is also interested in transitioning into a leadership and supervisory position as the territory develops.


About the Role:

This position begins with a strong focus on direct patient care in post-acute and long-term care settings. Over time, as the territory grows, the physician will take on more administrative and leadership responsibilities, including oversight of Nurse Practitioners (NPs) and Physician Assistants (PAs).


Responsibilities:

  • Provide comprehensive care for patients in Skilled Nursing Facilities and nursing homes.
  • Manage chronic conditions and acute changes in status to help prevent rehospitalizations.
  • Conduct rounds, coordinate with facility staff, and communicate with families and caregivers.
  • Collaborate with interdisciplinary teams including nurses, therapists, and case managers.
  • Ensure quality of care and compliance with all state, federal, and facility regulations.
  • As territory develops: supervise and mentor advanced practice providers (NPs and PAs).
  • Support safe discharges and continuity of care through effective care transition planning.


Qualifications:

  • MD or DO degree with Board Certification/Eligibility in Internal Medicine or Family Practice.
  • Active, unrestricted Florida medical license (or eligibility).
  • Prior experience in Skilled Nursing Facilities, long-term care, or post-acute medicine preferred.
  • Strong clinical decision-making with a focus on reducing unnecessary hospital admissions.
  • Excellent leadership, communication, and collaboration skills.
