Physical Therapist (PT)

Schedule Requirement: Must be available a minimum of 3 days per week
The Physical Therapist (PT) is responsible for evaluating, planning, and implementing individualized treatment programs to restore, maintain, and improve patient mobility, function, and overall well-being. The PT works collaboratively with physicians, nurses, and other rehabilitation professionals to deliver high-quality, evidence-based care in accordance with Connecticut Department of Public Health and American Physical Therapy Association (APTA) standards.
The ideal candidate demonstrates compassion, strong clinical reasoning, and a commitment to helping patients achieve optimal physical function and independence.
Key Responsibilities-
Perform comprehensive evaluations to assess strength, range of motion, balance, posture, coordination, and pain levels.

Develop and implement individualized treatment plans to improve physical function and mobility.

Provide therapeutic exercises, manual therapy, gait training, balance activities, and patient education.

Use modalities such as ultrasound, electrical stimulation, and heat/cold therapy as appropriate.

Reassess patient progress regularly and adjust care plans as needed to achieve optimal outcomes.

Educate patients and caregivers on home exercise programs, safety, and injury prevention.

Collaborate with physicians, nurses, and other rehabilitation staff to ensure coordinated care.

Accurately document evaluations, treatment notes, and progress reports in compliance with HIPAA and Connecticut DPH regulations.

Maintain therapy equipment and treatment areas in a safe and clean condition.

Participate in staff meetings, continuing education, and quality improvement initiatives.

Active and unrestricted Connecticut Physical Therapist License.

Graduate of an accredited Physical Therapy program (Doctorate or Master’s in Physical Therapy required).

Current CPR/BLS certification.

Minimum 1–2 years of clinical experience preferred (new graduates welcome to apply).

Strong knowledge of anatomy, kinesiology, and evidence-based rehabilitation practices.

Excellent communication, organization, and documentation skills.

Must be available at least 3 days per week, with flexibility for scheduling as needed.

Experience in home health, outpatient, inpatient, or long-term care settings.

Familiarity with electronic health record (EHR) systems.

Knowledge of Medicare and insurance documentation requirements.
