Physical Therapist

Harlem Center for Nursing and Rehabilitation is looking for a Physical Therapist to join the team at our Skilled Nursing Facility in Harlem, NY!

Physical Therapist Responsibilities

• Screen, evaluate, and treat residents per MD order
• Develop effective treatment plans
• Supervisor Physical Therapy Assistants and Rehab Aides in resident care
• Communicate with Supervisor and interdepartmental personnel regarding resident progress, problems, and plans
• Record clinical documentation according to accepted regulatory, facility, and professional guidelines
• Participate in scheduling of residents and discharge planning
• Secure necessary medical equipment and adaptive equipment for residents to facilitate independence
• Comply with state regulations and national standards and Allure Rehabilitation Services policies and procedures as they apply to the practice of Occupational Therapy

Physical Therapist Requirements

• Degree from American Occupational Therapy Association Accredited School of Occupational Therapy
• Current, valid NYS license
• COVID-19 vaccination

Preferred

• Previous experience in post-acute/long-term care
