Department: Community Mental Health Treatment Centers
Summary: We are seeking credentialed Peer Support Specialists for our Home and Community Mental Health Treatment Centers located in Brooklyn. Services are provided on and off site (home or any community setting). The Peer Specialists will provide recovery-oriented interventions for adults and children diagnosed with severe and persistent mental illnesses; accompany clients to appointments; and facilitate and co-facilitate outreach activities. They will be a NYS Certified Peer Specialist or Certified Recovery Peer Advocate (CRPA) with at least one year of experience.
Job Summary:
- Conduct phone and field outreach to help facilitate and develop strong connections with clients and families in need of services.
- Conduct routine monitoring of patient experiences through assessments and engagement.
- Relay peer issues of concerns to the treatment team and clinical leadership as appropriate.
- Complete WRAP plans, Safety plans and prevention plans with clients.
- Provide individual peer support and wellness counseling for clients in need. Conduct peer run support groups as appropriate.
- Assist in linking clients to services that will assist them toward meeting their treatment goals.
- Play an active role in the development and promotion of the community advocacy board
- Educate clients and consumers about the agencies services and benefits.
- Escort clients to appointments as needed.
- Work with clients and their treatment teams to help clients gain insight into symptom management, and coping skills to support them in their recovery process.
- Document all client contacts within the electronic health records within a timely fashion.
Qualifications:
- New York State Certified Peer Specialist or provisional certification provided permanent certification is accomplished within (6) six months of hire
- Certification or provisional certification from Howie the Harp, NYS Academy of Peer Services or other accredited provider (if provisional, to be completed within 6 months of hire.)
- Must have experienced mental health concerns themselves and/or have supported family members through their mental health experiences
- Commitment to Recovery-Oriented Practice
- Positive attitude and professional demeanor
- Ability to complete work independently as well as in collaboration with team members
- Must be team oriented with a willingness to be flexible and helpful
- Ability to travel daily into any area of Brooklyn and vicinity
- Effective written and oral communication skills
- Computer proficiency in Microsoft Office a MUST
- Ability to learn Electronic Health Record System
- Highly organized, motivated self-starter
- Excellent time management skills.
- High School Diploma or GED
- Spanish Speaking A PLUS
